Obtaining and completing Parent/Guardian Permission Form

The first step for granting permissions for your child is to print out the Permission Form. Obtaining a Parent/Guardian Permission From is a quick and easy process. Simply link here and print out the form itself.

Next, fill out the form and mail it or fax it to the address provided on the form.

Once we process the permission form, your child will receive an email with a temporary user name and password. The email will include instructions on how to log in to the website to complete their online registration. As the child's parent/guardian, you will also receive an email with a user name and password which will allow you to set up an online membership as well. Because of this, it is important that your email address be different than your child's. By setting up an online membership yourself, you can view or update the access your child has to the various activities on www.nintendo.com.
