Nintendo Parent/Guardian Permission Form Overview

Registering for My Nintendo is the easiest way to take advantage of everything Nintendo has to offer on our websites.  Nintendo provides a wide variety of online activities for players of all ages including cool interactive games, exclusive newsletters and much, much more.  Be sure to keep an eye on Nintendo.com for all of the latest activities and promotions!

As part of the registration process for a My Nintendo membership, we require certain information from each participant as listed on the following form.  Before collecting this information from children 12 and under, we require parent/guardian permission.  To grant your child permission to register for a My Nintendo membership, please complete the permission form and send it to the address provided at the bottom of the form. 

For this program, we require that the e-mail addresses for parents/guardians and their children be different.  This is because once we process the permission form, your child will receive an e-mail with a temporary user name and password that will allow your child to log in to the website to complete their registration.  As the child's parent/guardian, you will also receive an e-mail with a user name and password which will allow you to change or update the access your child has to the various activities on www.nintendo.com.

In addition, we would like you to be aware that there are two types of surveys that may be conducted on Nintendo.com.  One is where anonymous demographic or opinion information is requested that will not be tied to a participant’s My Nintendo profile.  In these cases, parental permission is not required for a child 12 and under to participate.  In some cases we may conduct surveys where the information collected will be associated to your child’s My Nintendo profile.  Parental permission is required under these circumstances in order for a child to participate.

Nintendo of America Inc. respects the privacy rights of our online visitors and recognizes the importance of protecting the information collected about our customers.  We have adopted a corporate-wide Online Privacy Policy that guides how we collect, store and use the information provided to us.  A copy of our Privacy Policy can be found at www.nintendo.com/corp/privacy.jsp.

If, for any reason, you are concerned that the personally identifiable information maintained by Nintendo of America Inc. is inaccurate, or wish to verify or delete the information we have in our records, please contact us by e-mail (privacypolicy@noa.nintendo.com), by fax (425-558-7300), or by phone (888-977-7627).  We will be happy to review, update, or remove the information as appropriate.  Keep in mind that children’s personal information is always considered highly sensitive.  As such, we take steps to verify the identity of anyone requesting access to personal information in our files.  Please be sure to have verifying information available when contacting our Online Administrators.  To protect your privacy, no specific information will be given out without proper verification.
